Southeast Primary Care Partners/Southeast Medical Group (SEMG) is seeking a dedicated and dynamic Quality Specialist to join our team.
This pivotal role serves as the crucial link between our clinic's lead physician and the SEMG's population health team, with a focus on driving and improving quality outcomes for our patients.
The ideal candidate will be instrumental in executing key initiatives aimed at enhancing performance on quality measures, actively participating in monthly quality review meetings, and identifying patients for enrollment in SEMG's clinical programs.

Responsibilities:
Liaison Activities:
Act as the primary liaison between the clinic's lead physician and the SEMG's population health team to facilitate improved quality outcomes.
Quality Initiatives Execution:
Implement and monitor key initiatives within the clinic to enhance performance on quality measures.
This includes outreach to patients overdue on care gaps, coordinating with other providers to gather medical records, and scheduling necessary appointments.
Monthly Quality Reviews:
Engage actively in ongoing monthly quality review meetings with providers and the population health team, aimed at identifying and leveraging opportunities for improvement.
Patient Enrollment:
Identify patients eligible for and assist in enrolling them into SEMG's clinical programs, such as care management and remote monitoring services.
Independent Functioning:
Operate independently based on overall team priorities, including prioritizing call lists and developing innovative approaches to drive quality outcomes in partnership with lead providers.
Background and
Qualifications:
Education:
Medical Assistant (MA) certification, ideally obtained by the first day of employment or within the first 6 months.
Experience:
Previous experience in value-based care or with quality outcomes is strongly preferred.
Skills:
Exceptional ability to function independently, prioritize effectively, and develop new strategies to achieve quality outcomes.
Strong communication skills to effectively liaise between different teams and coordinate with external providers.
Personal Attributes:
The candidate should be proactive, detail-oriented, and have a strong commitment to healthcare quality and patient care.
